No PHI. NexLookup does not collect, store, or transmit protected health information.

If you create an account, we store your username, email, password hash, and optional profile fields. Search history and favorites are stored only for your account and can be cleared at any time.

NPI registry queries are sent to the public CMS NPPES API; we do not retain NPI search results.

Server logs may include IP addresses and request paths for security and rate limiting. We do not sell or share personal data.
